FORM FOR MAKING A REPORT PURSUANT TO LEGISLATIVE DECREE 24/2023 (SO-CALLED “WHISTLEBLOWING”)

This form can be filled out to make a report pursuant to Legislative Decree 24/2023, as outlined in the information document prepared by the organization regarding whistleblowing and the privacy notice available on the organization’s website, in the section dedicated to whistleblowing, which is referenced. The report will be handled according to the legal provisions and the guidelines set by ANAC in this regard, in order to ensure compliance with personal data protection regulations.

The form consists of fifteen points, spread across six pages. On the last page, the reporter is asked to provide their signature (unless the report is anonymous). A copy of the reporter’s identity document must be attached to this form (unless the report is anonymous).

Note: By submitting the report, the reporter declares that they have read the above privacy notice on the processing of personal data, in accordance with Articles 13 and 14 of EU Regulation 2016/679, as well as the informational document published in this regard.

However, anonymous reports or reports that fall outside the scope of the regulations will be considered ordinary and handled as such, according to the organization's regulations, unless otherwise provided by Article 16, paragraph 4, Legislative Decree 24/2023.

Note: The following cannot be subject to reporting, public disclosure, or denunciation:


  • Complaints, claims, or requests related to a personal interest of the person reporting or the person who has filed a complaint with the judicial or auditing authority, which solely concern their individual labor or public employment relations, or related to their labor or public employment relations with their hierarchical superiors;


  • Reports of violations already regulated mandatorily by the acts of the European Union or national laws indicated in Part II of the annex to the decree, or by national laws implementing the European Union acts listed in Part II of the annex to Directive (EU) 2019/1937, even if not specified in Part II of the annex to D.L. 24/2023;


  • Reports of violations in matters of national security, as well as contracts related to defense or national security aspects, unless such aspects fall within the relevant EU derived law.

Illicit act falling under EU acts on public procurement, financial services/markets, anti-money laundering/anti-terrorism, product/transport safety, environmental protection, radiation protection/nuclear, food safety/animal protection, public health, consumer/data/privacy protection, network/information system security.
Acts or omissions that harm the financial interests of the Union;
Acts or omissions concerning the internal market (by way of example: violations related to competition and state aid);
Acts or behaviors that undermine the purpose or objective of the provisions laid out in the Union acts.
